RESIDENCY REQUIREMENTS:

• Texas pharmacist licensure must be in good standing by 30 days after the start of the residency
• Staff every third weekend
• Staff at least one major and one minor holiday
• Start Residency around July 1st 
• One major and one minor project
• Manuscripts on projects
• Code Blue participation
• On-call responsibility
• Case Presentations, seminars and journal club presentation
• Completion of solid organ transplant topics checklist
• Completion of 5 medication error reports
• Validation for warfarin and vancomycin consults
• Submission of a major project to a transplant-related conference
• Prepare or revise a transplant protocol
• Successful achievement of 80% of learning objectives
• Completion of residency portfolio

 

APPLICATION PROCESS:

You must apply online through the ASHP PhORCAS program (www.ashp.org/phorcas) (application documents are not to be mailed).

The following documents are required to be submitted as part of a complete application for the PGY2 Solid Organ Transplant Pharmacy Residency.  The deadline for application is January 5th, 2024 at 11:59 PM.

1. Cover Letter -- please provide a cover letter for your application materials which addresses:  a) your short and long term career goals, b) your thoughts on how a solid organ transplant residency will help you achieve these goals, c) why you feel that a specialty residency is now the best career path for you, and d) any information about yourself and your candidacy which may not be apparent on other documents.
2. A copy of your curriculum vitae (CV). 
3. Most recent College of Pharmacy and/or Graduate School transcript(s). This is waived if you are more than five years post-graduation. The transcript can be an unofficial copy.
4. Letters of Recommendation:  Three (3) letters of recommendation are requested with at least two from your PGY1 pharmacy practice residency program. No set format is required for the letters; they need to address your potential for success in the residency program.  The ASHP-developed recommendation form must be included with the letters.
5. A passport size photo required to be submitted after offer to interview is accepted. 

A virtual interview will be required to complete the application. All applications will be screened by our Residency Advisory Council, and all applicants chosen for interview will be contacted by the end of January. The virtual interview will be a partial day consisting of a patient case and interviews with the transplant clinical pharmacist team and clinical manager as well as transplant residents. The last date for interviews is normally one week before the ASHP match list submission date.
